Skilled Nursing Facilities in Nevada must complete a COVID-19 Daily Status Survey via a link sent in an email from the Division of Public and Behavioral Health (DPBH).

How Do I Report?

Complete the survey via a link sent in an email from the Division of Public and Behavioral Health (DPBH) Office of Analytics when there is not an active outbreak situation at the facility. If an email is not received, facilities should go directly to the survey link and continue to report daily.
