Reporting Information

In September 2018, NRHP, DPBH and Bitfocus developed a legal hold reporting form for use by NRHP members as part of a pilot program to track legal holds and address the delays in placement of these patients.

Method

How Do I Report?

All participating members have a single login assigned to a staff member that is used for all facility reporting. The process is outlined below:

Intake Process: Login and search for an existing patient records
Step 1: Search for the patient - On the search bar, enter the first three letters of the patient’s first and last name. Confirm search results with date of birth and/or social security number before selecting patient. You can also search directly on the search bar with the DOB or SSN.

Step 2 : If the Patient is found - mouse over their name and select the “edit” icon to open the file. Skip to step 5.

Step 3: If NOT found - If, after a thorough search, you have determined that your patient does not exist in CMIS, click the “Add Client” icon in the upper right corner of the Search screen.

Step 4: Complete the Patient’s Profile – Enter/Update all profile screen data to the best of your ability. Select “Add Record” when you are finished to save the patient’s profile.

Step 5: Accessing Forms: To access the NV Legal 2000 Tracking Form from the patient profile screen, click on the Assessments tab at the top of the screen.

You will arrive at the “Assessments” screen where you will see the form as well as the patient’s “Assessment History”. The patient’s assessment history is a comprehensive list of every assessment they have had by CMIS participating organizations since having been entered into CMIS.

Step 6: Completing Forms – To gain access to the forms simply click the “start” button next to the form you wish to complete. You will arrive at the desired form where you can begin filling in the associated questions. Once the form is complete, click “Save” at the bottom of the page. To double check that the form has been saved, click on the “Assessments” tab at the top of the screen. You
should now see that completed form under “Assessments History”.

Step 7: Editing your form – If you find that you need to edit a form, go into the Assessments tab, find the form of interest under “Assessment History”, mouse over the name of the form, and click on the “edit” icon. This will bring you back to the form and allow you to add/delete information.

Step 8: Return to the Search screen – Click “Search” in the upper right corner to return to the main search screen to begin the process for the next patient.

Reporting this data helps NRHP monitor any issues with placing legal holds, is used to validate the state data and to address trends as they occur.

The legal mandate requiring reporting

NRHP has entered into a BAA with Bitfocus in order to get de-identified data exported and uses this dataset to develop dashboard reports. This Business Associate Addendum (“Addendum”) supplements and is made a part of the contract (“Contract”) by and between NRHP on behalf of its Member Hospitals, each of which shall be referred to herein collectively as Covered Entity (“CE”) and Bitfocus, the Business Associate (“BA”), dated July 20, 2018. This Addendum is effective as of October 11th, 2018 (the “Addendum Effective Date”).
